野兎病 Tularemia は、細菌 the bacterium _Francisella tularensis を原因とする人獣共通感染症 a zoonotic disease で、様々な動物保有種が確認されている a wide range of animal reservoirs が、もっとも多いのはウサギ目 lagomorphs (i.e., rabbits and hares) とげっ歯類 rodents である。
感染経路は、感染動物との接触、菌が混じった水の飲用、エアロゾル内の細菌の吸入などである。
臨床症状は、曝露形式により異なる症状が現れる。抗生物質による治療で、多くの患者は完治する。
In Washington, fewer than 10 human cases are reported annually.
Investigation and Outcomes
2023年10月23日、ワシントン州在住 Kitsap County, Washington の生来健康であった32歳女性が、左手の有痛性腫脹のためプライマリケア医を受診した。この女性は野生動物の専門家 a wildlife biologist for a nonprofit organization で、海峡沿い along South Puget Sound で死亡した発見されたアザラシ a harbor seal (_Phoca vitulina_) を解剖した10月3日に指を切ったことを報告した。感染防護衣は着用していたが、手袋とともに指を切ってけがしていた。当初治癒すると思われたが、2週間後に炎症と痛みが生じ、微熱、リンパ節腫脹、咳嗽、充血などの症状も現れた。ドキシサイクリン doxycycline と外用薬 topical mupirocin が処方され完治した。
治療医は野兎病 tularemia を疑っていなかったが、創部浸出液を提出し、培養検査を行った検査機関で野兎病菌 suspected _Francisella_ species が疑われた。11月3日に the Washington State Public Health Laboratory に分離菌が届き、野兎病菌と確認された tested positive for _F. tularensis_ by bacterial culture, direct fluorescent antibody, and polymerase chain reaction (PCR)...

[Mod.LL－ Tularemia is a bacterial infection that affects, aside from people, more than 250 species of wild and domestic mammals, birds, reptiles, and fish. It is listed as a category A bioterrorism agent because of the potential for fatality, airborne dissemination, and societal disruption if released. The causative bacterium, _Francisella tularensis_, is a non-spore-forming, Gram negative coccobacillus antigenically related to _Brucella_ spp.
This human case of ulceroglandular tularemia is the 1st known report of tularemia acquired through contact with a marine mammal, and the 1 detection of _F. tularensis_ in a marine mammal.
Tularemia can be transmitted by aerosol, direct contact, ingestion, or arthropods. Inhalation of aerosolized organisms (in the laboratory or as an airborne agent in an act of bioterrorism) can produce a pneumonic form. Direct contact with, or ingestion of, infected carcasses of wild animals (such as cottontail rabbit) can produce the ulceroglandular, oculoglandular, oropharyngeal (local lesion with regional lymphadenitis), or typhoidal form. Immersion in or ingestion of contaminated water can result in infection in aquatic animals. Ticks can maintain infection transstadially (pathogen remains with the vector from one life stage ["stadium"] to the next) and transovarially (transmission of a pathogen from an organism [as a tick] to its offspring by infection of eggs in its ovary), making them efficient reservoirs and vectors]

Toads は、皮膚、唾液腺、内臓、卵などに toxins を含んでいることがあり、Toad venom 毒により摂取後数時間で危険な消化器、呼吸器、神経などの症状を発症する。嚥下により血流に入ると、心血管に最も危険な症状が現れる。重症例では、肝不全、急性腎不全から死に至る場合がある。

Abstract
"Objective: This study was performed to describe the clinical characteristics and outcomes of patients with toad poisoning in Thailand.
"Methods: 過去5年間 (2012-2016) の中毒センターのサーベイランスシステム the Ramathibodi Poison Center Toxic Exposure Surveillance System データについて、中毒患者の後ろ向き研究 a retrospective study of patients with toad poisoning を行った。
"Results: 36 patients のヒキガエル毒 toad toxin の中毒が確認された。年齢の中央値は31歳で、66.7%が男性で、50％がヒキガエル肉を摂取していた。
最も多く見られた症状は、摂取の平均2時間後に発症する消化器症状 gastrointestinal (GI) symptoms だった。12例に徐脈がみられ、7例がショック症状となり、1例に心停止が起きた。
すべての患者の心電図 the initial EKGs of all patients で最も多い所見が洞徐脈 sinus bradycardia であった。
病初期（救急室収容後15分以内または摂取後4.5時間以内）の治療中に2例の患者が心停止となった。入院中に、1例が洞徐脈、2例が徐脈性不整脈 bradyarrhythmia を発症した; しかしいずれも状態は安定していた。心室性頻脈などの頻脈性不整脈はみられなかった No tachyarrhythmias 。11.1%に高カリウム血症、5例にジゴキシン Serum digoxin, ranging from 0.43 to > 8 ng/mL が認められた。75％が平均2日間（0.5-5日間）入院した
全体の致死率 The overall mortality rate は8.3%で、死亡患者はすべて toad meat and/or eggs を食べ心停止 cardiac arrest を発症していた。
