南部 Ca May Province の生後1日目の新生児が、先天性梅毒予防のためのペニシリン注射のあと死亡したと、Ca Mau 産科小児科病院が23日明らかにした。
21日に妊娠39週で帝王切開により生まれ、体重は3.7kgだった。生下時は問題なかったが、Dam Doi District 原住民の母親が梅毒検査で陽性であったため新生児病棟に入院し、翌日 benzathine benzylpenicillin at a dose of 50 000 units/kg body weight の筋肉注射が行われた。しばらくしてチアノーゼ、陥没呼吸、呼吸困難となり、酸素投与とアドレナリンの筋肉注射等の蘇生処置が行われたが、重症アナフィラキシーショックのため死亡した。

髄膜炎菌 meningococcus に感染した37歳男性が、ホーチミンの熱帯病病院 the Hospital for Tropical Diseases in Ho Chi Minh City で治療を受けている。24日、10日前から入院中であると病院が明らかにした。意識低下、高熱、悪寒、激しい頭痛、関節腫脹と関節痛、暗紫色の発疹などの症状から、髄膜炎菌性髄膜炎 meningococcal meningitis と診断され高用量の抗生物質で治療された。

エーリキア症 _Ehrlichia chaffeensis_ ehrlichiosis はダニ媒介性細菌感染症 a tickborne bacterial infection で、米国南東部と南中部に常在 endemic to the southeastern and south-central United States するダニ lone star tick (_Amblyomma americanum_) により伝播される。
臓器移植関連の感染伝播はまれであるため、ドナー臓器のエーリキアのスクリーニングは行われていないが、レシピエントの感染の可能性がある。
We report a case of donor-derived _E. chaffeensis_ ehrlichiosis in a Wisconsin, USA, resident from a living kidney donor in Minnesota, USA.
The Study
臓器提供者 ドナー The living donor は、a 33-year-old man with obesity and unremarkable preoperative examination and serologies who underwent laparoscopic hand-assisted right nephrectomy for National Kidney Registry living unrelated kidney transplant donation at the University of Minnesota (Minneapolis, MN, USA) in June 2023.
ドナーの腎摘出術は合併症なく an uncomplicated nephrectomy 4時間29分で終了したが、術当日に臀部と下半身の新たな紅斑 a new erythematous rash の出現があり、筋肉痛を伴っていた。尿量の減少があり、褐色尿となり. Creatine kinase (CK) level は 41 155 U/L (reference range 30-200 U/L) に上昇した。横紋筋融解症 Rhabdomyolysis と診断され積極的輸液 aggressive fluid replacement が行われた。尿中ミオグロビン Urine myoglobin and CK levels は徐々に改善し、6病日目に退院した。
レシピエントは a 24-year-old man with end-stage kidney disease secondary to IgA nephropathy で、腹膜透析 peritoneal dialysis が実施されていた。
腎移植 a National Kidney Registry transplant from the described unrelated donor to the right iliac fossa with antithymocyte globulin induction and peritoneal catheter removal at the University of Wisconsin (Madison, WI, USA) を受けた。
術後経過は良好で、3病日目に退院し、a creatinine level of 1.8 mg/dL (reference range 0.73-1.18 mg/dL) であり、maintenance immunosuppression (mycophenolate mofetil, tacrolimus, prednisone) and antimicrobial prophylaxis (valganciclovir, trimethoprim/sulfamethoxazole) を処方された。
1週間後に38.2℃の発熱のため再入院となり、全身倦怠、関節痛、腎周囲の液体貯留 a perinephric fluid collection measuring 4.9 x 5.1 x 3.8 cm with 69 nucleated cells, predominately macrophages を伴っていた。
抗生物質 empiric intravenous piperacillin/tazobactam and vancomycin for 48 hours が投与されたが、細菌培養が陰性であったため48時間後に中止された。
39.4°C まで上昇し、腎機能は悪化し、好中球と血小板減少を認めたため、バベシア、エーリキア、アナプラズマの検査 broad-range 16S rRNA gene PCR on the perinephric fluid, serum Lyme PCR, whole-blood _Babesia_ spp. PCR, and whole blood _Ehrlichia_ and _Anaplasma_ spp が実施された。急速に腎障害進行したため，a graft biopsy が実施された。患者の詳しい病歴で，no new sexual partners, no international travel, and minimal outdoor exposure before or after transplantation が確認されており，男性にも飼い犬にもダニの曝露 known recent tick exposures はなかった。

Equateur Province in the Democratic Republic of Congo (DRC) における、2件の原因不明のクラスターについての検査で、エボラ Ebola and マールブルグ Marburg viruses は否定され、さらに検査が続いていると the WHO African regional office in its latest weekly outbreak and health emergencies update で述べられている
この最新の集団発生は、[Bomate Village] in Basankusu Health Zone を中心に発生しており、2月9日に初めて州当局に報告された。当初32 cases , including 20 deaths in the community と報告されていたが、同15日に a total of 419 suspected cases and 45 deaths と報告されている。
死亡の半数が発症から2日以内に起きており、症状として、発熱、頭痛、体性痛、頸部硬直、咳嗽、胃腸炎症状がみられている。
13 patients について the National Institute of Biomedical Research (INRB) in Kinshasa で検査が行われたが、すべて陰性 negative for Ebola and Marburg viruses だった。
"検索されている鑑別診断 Differential diagnosis under investigation は、マラリア malaria, ウイルス出血熱 viral haemorrhagic fever, 食（水）中毒 food or water poisoning, 腸チフス typhoid fever, and 髄膜炎 meningitis" と the WHO は述べた。
Ebola and Marburg viruses produce hemorrhagic (bleeding-related) fevers.
その前のアウトブレイク The earlier unexplained outbreak が初めて報告されたのは January 21 [2025] で、Boloko Village in Bolomba Health District を中心に発生し、1月に3例の小児の死亡 Three community deaths in children が報告され、患児らはコウモリの死体に触れ、出血症状があったとされている。子供等の間で同様の症状が報告され、Boloko Village では12例のうち8例がが死亡、nearby Danda Village でも2例の患者と1例の死亡が報告された。the provincial and INRB labs の検査では陰性 negative for Ebola and Marburg viruses であった。
"The exact circumstances of exposure have not yet been established in both outbreaks. Additionally, no epidemiological links have been established between the cases in the 2 affected health zones," the WHO said.
Equateur Province is in the northwestern part of the DRC and has been affected by Ebola outbreaks in the past.

ASF-like ailment kills over 100 hogs in Northern Samar
The municipal government of Catarman, the capital of Northern Samar, has reported that over 100 pigs from 50 backyard raisers had died after displaying symptoms of the African swine fever (ASF). The municipal information officer said they were yet to confirm if the pigs died due to ASF, "although they displayed ASF-like symptoms," like high fever, skin redness, loss of appetite, vomiting, weakness, and cough, among others.
