Abstract
オロプーシェウイルス Oropouche virus が現在ラテンアメリカで重要な病原体となっている。
2024年5月27日，キューバ公衆衛生省 the Ministry of Public Health of Cuba からはじめてアウトブレイク the first ever outbreak of Oropouche fever. In が報告された。
今回，3例のギラン・バレー症候群と診断された患者 human cases of Oropouche virus infection with symptoms and signs of neurological disease and clinical diagnosis of Guillain-Barré syndrome について報告する。
Brief report
... 2024年5月24日，the Ministry of Public Health of Cuba がはじめての感染例 the first ever outbreak of Oropouche virus disease を報告した。A total of 74 confirmed cases were reported from Province of Santiago de Cuba (n = 54), and from Province of Cienfuegos (n = 20) が報告されている。
In this brief report, we describe 3 human cases of OROV infection with symptoms and signs of neurological disease and clinical diagnosis of Guillain-Barré Syndrome (GBS).
Case 1
気管支喘息と胆石の既往のある San Luis, Santiago de Cuba province 在住の53歳女性が，6月7日に発熱，下痢，全身倦怠，頭痛，多関節痛を発症し，6日後に回復した。同月17日に両下肢の筋けいれん cramps と筋力低下 weakness in both lower limbs が現れ次第に歩行困難となった。4日後に口角偏位と閉眼困難となり救急受診し，精査治療のため入院した。意識・見当識に異常なく，末梢顔面神経麻痺と運動失調，深部腱反射消失が認められた。髄膜刺激症状は認められなかった。
Case 2
高血圧症と骨粗しょう症のある Santiago de Cuba city, Santiago de Cuba province の64歳女性が，6月7日から高熱，悪寒，頭痛，関節痛，4日間の腹痛があり，11日後に右優位の手足のしびれ，頭痛，筋力低下を発症した。救急受診後入院となった。
意識見当識に異常なく，高血圧，歩行障害，筋力低下，dysarthria がみられたが，髄膜刺激症状は認められなかった。右半身弛緩性麻痺等 direct right flaccid hemiparesis with facio-brachial predominance, static taxi by motor deficit, and right central facial paralysis がみられた。
Case 3
高血圧と腎臓病の持病のある，Santiago de Cuba city, Santiago de Cuba province 在住の51歳男性が，6月9日に発熱，全身倦怠感，下痢，関節痛の症状を発症し，5日後に改善した。同月19日に，cramps or tingling in his fingers and toes, back pain mainly in the lumbosacral region, muscle weakness in lower limbs, which was progressing and began to have difficulty walking. The muscle weakness also affected the upper limbs. He went to the emergency room and was admitted for study and treatment. Clinical examination revealed he was conscious, time, space, and person oriented with muscle weakness, motor deficit, and diminished deep-tendon reflexes. No signs of meningeal irritation were detected.
すべての症例 all 3 cases で，発症から10日ないし12日後に筋電図 electromyography と神経伝達速度 nerve conduction studies 検査が行われた。
検査の結果，ギラン・バレー症候群に矛盾しない急性運動ニューロパチー症 an acute motor axonal neuropathy compatible with the clinical diagnosis of GBS の所見がみられ，complementary clinical and microbiological laboratory tests [8,9,10,11] were indicated.
Other microbiological diagnostic tests were performed. In all cases, CSF were examined for influenza virus A, B and C, human adenoviruses, human respiratory syncytial viruses, human parainfluenza virus 1-4, coronaviruses, rhinoviruses, enteroviruses, and human herpesviruses [12,13]. In addition, Rapid HIV test and serology test for syphilis were also done. The results in all cases were negative.

The Israeli Ministry of Agriculture and Food Security said on Wednesday [18 Sep 2024] that it has detected an outbreak of highly pathogenic avian influenza, or bird flu, in the northern Gilboa region.
The outbreak was discovered on a turkey farm, which houses 34 000 birds, in the village of Ram-On, north of the West Bank, the ministry said in a statement.
