14年前からマラウイに滞在し現在 Lilongwe 在住の米国民である 36歳の男性教師が，東アフリカトリパノソーマ症 East African trypanosomiasis (EAT) と診断された。6月初めに the Nkhotakota Wildlife Reserve で釣りをしたが，ツェツェバエの刺咬の記憶はない。この場所は EAT が発生することで有名。この 12日後に急性の発熱があり，当初は細菌感染症，次にマラリアの治療を受けていたが，マラリア迅速診断検査が陰性であったにもかかわらず，血液塗抹標本で gametocytes が報告されていた。アーテスネートの静脈注射 IVI [intravenous infusion] artesunate の効果がないことに医師は悩んでいた。血液標本の再検でトリパノソーマ trypomastigotes が確認された。Kasungu [Central region of Malawi] での suramin treatment が検討されたが，血小板数の減少があり，19日に a Johannesburg [South Africa] hospital への搬送が決定された ...
