Measles warning in Victoria and NSW as new cases linked to travel from Bali and other Asian destinations

3月7日にクリニックを訪れた32歳のドイツ人女性は、両側の足関節と中足骨関節、および中手骨関節の痛み persistent arthralgia を訴えていたが、受診時に発熱はなかった。
発症は1月14日で、12月下旬にドイツからの直行便でスリランカに到着し、スリランカ滞在中に40℃の発熱と関節痛があったが、発疹は認めなかった。
1月15日に現地のクリニックを受診した診療記録によると、デング熱は除外診断され、チクングニア chikungunya IgM 抗体は陰性 negative とされているが、検査結果は見当たらなかった。
当クリニックで行ったチクングニア血清検査 chikungunya serology using ELISA では、Ig M G 抗体とも著明に上昇 markedly elevated levels of both IgM and IgG antibodies (IgM: 4.05 [reference value <0.8]; IgG: 4.11 [reference value <0.8]) していた。
デング熱とジカウイルス検査 Serological tests for dengue and Zika viruses (IgM and IgG, respectively) は陰性 negative だった。
抗核抗体とリウマチ因子 Antinuclear antibodies (ANA) and rheumatoid factors were also negative.
Additionally, white blood cell count, erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) levels were within normal limits.
渡航歴と臨床症状からチクングニア熱と診断した。
Given her travel history to an area with known chikungunya virus transmission and the clinical presentation, these findings are consistent with a chikungunya virus infection.

神経梅毒 Neurosyphilis, 眼球梅毒 ocular syphilis, and 耳梅毒 otic syphilis (NOO syphilis) caused by _T. pallidum_ ではそれぞれ、中枢神経、眼球、耳に波及し、どの病期 the primary, secondary, latent, or tertiary stages of syphilis でも発生する。NOO syphilis ではしばしば debilitating の状態となり、髄膜炎、卒中、運動・感覚脱失 motor or sensory deficits、失明、聴力損失の原因となることもある。
2000年以降のデータ Data from the early to mid-2000s によると、NOO syphilis は among gay, bisexual, and other men who have sex with men (MSM) and persons with HIV に多くみられるとされている。

2019年に比べ、During 1 Jan - 31 Oct 2023 の期間において、シカゴの NOO syphilis cases in Chicago は HIV 感染がなく、MSM ではない集団でより多く認められた。医療従事者は、たとえ HIV 感染がなく、MSM でなかったとしても、NOO syphilis を考慮すべきである。
Enhanced surveillance efforts to better understand NOO syphilis trends are needed.
