2018年11月17日，Atascosa County, Texas の 63歳男性が，3日間続く発熱，胸痛，呼吸困難の症状のため A病院を受診した。入院時に先天性の片腎無形成と未診断の2型糖尿病が指摘されている。胸腹部の造影 CT 検査で，少量の左胸水を伴う左下葉肺炎が指摘された。intravenous azithromycin and ceftriaxone による治療が開始された。血液培養の結果，類鼻疽菌 _B. pseudomallei_が疑われ，確認検査のため検体が送付された。
11月20日，前胸壁中央部に限局性の淡紫色皮膚病変を生じ，次第に膿性となり潰瘍を形成した［写真あり］。翌日に呼吸不全となり緊急気管内挿管の上，B病院に転院となった。B病院には疑い診断名が伝えられておらず，血液及び胸部創の培養が行われ，いずれもグラム陰性桿菌が確認され，血液検査で急性腎障害が示された。同 25日に trimethoprim/sulfamethoxazole and ceftazidime 感受性を示す類鼻疽菌が同定され，ceftazidime による治療 (by using dosing for continuous renal replacement therapy) に切り替えられた。
