Outbreak at a glance
11月12日 インドネシア保健省は，Pidie district in Aceh province の循環するワクチン由来 2型ポリオウイルスによる麻痺のある感染例 a confirmed case of circulating vaccine-derived poliovirus type 2 (cVDPV2) with acute flaccid paralysis (AFP，急性弛緩性麻痺) について WHO に報告を行った。政府及び地方の公衆衛生各局の調査と，発生地域における 13歳未満の小児に対する予防接種キャンペーンが開始された。
Description of the outbreak
11月12日に a confirmed case of VDPV2 が報告された。患児はアチェ州 Aceh province の 7歳の男児で，10月9日 AFP を発症した。経口および不活化ポリオワクチンのいずれも接種されておらず，渡航歴や渡航歴のある人々との接触歴もなかった。11月25日，同じコミュニティ内であるが確定例との濃厚接触のない 3人の健常児から採取された便検体の検査で，新たに 3例の遺伝学的関連性のあるウイルス genetically related isolates of cVDPV2 が分離された。Biofarma lab での遺伝子解析の結果，the AFP case では 25か所の核酸の変化が，the 3 asymptomatic children でも 25ないし26か所の変化が認められた。この結果は，ウイルスが感染伝播したことを示す証拠であり，circulating VDPV2 (cVDPV2) として分類するための定義を満たしている。過去においてインドネシアでは，Papua province で a cVDPV type 1 outbreak が 2019年に発生している。Aceh province は定期予防接種計画におけるポリオワクチン接種のカバー率が非常に低い; が，インドネシア国内ではほかにも低率の州が複数存在し，Aceh 州に近い 3 provinces (North Sumatera, West Sumatera, and Riau) もその中に含まれている。2021年の Aceh province の経口 2価ポリオワクチンのカバー率 bivalent oral polio vaccine (OPV3) coverage は 50.9% で，不活化ワクチン IPV は 28.2% であり，Pidie district については，the coverage was 17.7% for OPV3 and 0.5% for IPV であった。すべての型のポリオウイルスに対する集団免疫が低い状態にあるが，とりわけ経口ワクチンが 3価から 2価に切り替わった 2016年4月以降に生まれた小児の 2型ウイルス（で顕著）である。
