Kelowna [British Columbia] mother の 6歳女児が 3週間前に連鎖球菌性咽頭炎 an intense sore throat となり，家庭医を受診したところ，培養検査が行われ自宅で結果を待つよう指示された。4日後症状が悪化し，救急室 the Kelowna Urgent Primary Care Centre を受診した。連鎖球菌検査の結果が陽性であることが知らされ，救急室で抗生物質が処方され帰宅することとなった。服用を開始したがさらに状態が悪くなり，その日にもう一度救急室を受診した。一生の中で最悪の頭痛があった。救急室に駆け込んだが，咽頭炎による頭痛であるとして帰宅させられた。翌日，6回目の抗生物質を服用したが状態は改善せず，さらに悪化したため救急室を受診し，小児科医の診察を依頼したところ直ちに入院となり，あらゆる抗生物質が投与された。3種類の抗生物質が投与され，脳の腫脹に対する薬剤だった。血液検査で血液中の細菌が確認されたため，髄液を調べたところ髄液でも細菌が確認され，すなわち脳内に細菌が入ったことを意味した。

Situation at a glance
From 1 Nov 2022 to 27 Jan 2023 の期間に，合計 559例の髄膜炎 cases of meningitis (of which 111 are laboratory confirmed), including 18 deaths (overall CFR 3.2%) がニジェール南東部 Zinder Region, southeast of Niger から報告されている。during 1 Nov 2021 to 31 Jan 2022 の時点で報告されていたのは the 231 cases であった。確定診断された症例の大部分 The majority of laboratory-confirmed cases (104/111; 93.7%) が，C 群髄膜炎菌 _Neisseria meningitidis_ serogroup C (NmC) による感染であった。緊急の 3価ワクチン接種キャンペーン Reactive vaccination campaigns with the trivalent ACW meningococcal polysaccharide vaccine が行われている。

Description of the situation
the African meningitis belt にあるニジェールは，2015年以降これまでにも複数の髄膜炎流行が発生し，20 789 cases and 1369 deaths (CFR 6.6%) が報告されている。

The last meningitis outbreak in the Zinder region の発生は the 2021/2022 season, with a total of 372 cases, including 12 deaths (CFR 3%) であった。
感染が疑われる症例 suspected cases から採取された 228検体中，154 (67.5%) について Niamey's Center for Medical and Health Research (CERMES) で解析され，_N. meningitidis_ serogroup C（C 群髄膜炎菌） was identified in the majority of confirmed cases (n=104; 93.7%), followed by _Streptococcus pneumoniae_ (肺炎球菌，n=5; 4.5%) and _Haemophilus influenzae_ (インフルエンザ菌，n=2; 1.8%). The remaining 43 samples tested negative.
Among the total of 559 cases of meningitis,
アウトブレイクによる患者数が最も多い年齢は 20歳未満 (n=538; 96.3%) で, 10－14歳が 202 cases (36.2%), the 5-9 years age group with 153 cases (27.4%), the 15-19 years age group with 107 cases (19.1%), and the 0-4 years age group with 76 cases (13.6%) であった。
地域としては，The most affected health district of Zinder region is Dungass (342 cases, 6 deaths), followed by Matamèye (98 cases, 3 deaths), Mirriah (72 cases, 3 deaths), Magaria (38 cases, 5 deaths), Zinder ville (7 cases, 1 death), and Gouré (2 cases, 0 deaths).
Epidemiology of meningitis
髄膜炎 Meningitis は，脳と脊髄を包む膜である髄膜 the meninges の重篤な感染症で，様々な種類の細菌が髄膜炎の原因となるが，肺炎球菌，インフルエンザ菌，髄膜炎菌によるものが最も多く，感染のある患者の呼吸飛沫や咽頭分泌物を介してヒトからヒトに感染伝播される。
合計 12種類の血清群 serogroups of _N. meningitidis_ が確認されており，このうち 6 of which (A, B, C, W, X, and Y) が流行 meningococcal meningitis epidemics の原因菌となる。潜伏期間は平均 4日間だが，2日から 10日間の幅がある。髄膜炎で最も多い症状は項部硬直 a stiff neck, 高熱 high fever, 光線過敏 sensitivity to light, 錯乱 confusion, 頭痛 headaches, および嘔吐 vomiting である。たとえ早期に診断し適切な治療が行われたとしても，5% to 10% of patients が典型的には発症の 24から 48時間後に死亡する。細菌性髄膜炎では生存患者の 10-20% に，脳障害，聴力損失，学習障害などが残る。より少ないが深刻な（時に致死的）髄膜炎菌性疾患として，出血性発疹と急速な循環虚脱 a haemorrhagic rash and rapid circulatory collapse を特徴とする，髄膜炎菌性敗血症 meningococcal septicaemia がある。
この疾患の最も重い負荷がかかっている地域はアフリカのサブサハラ地域 a region of sub-Saharan Africa で, アフリカ髄膜炎ベルト the African meningitis belt と呼ばれ, 特に髄膜炎菌性髄膜炎のリスク地域として認識されているが，肺炎球菌性髄膜炎のハイリスク地域でもある。
ニジェールの国土の大部分が the African meningitis belt に位置し，（通常 1月から 6月にかけて）季節性パターンに従った髄膜炎流行が年ごとに規模を変えて発生する。
2015年に a large meningitis outbreak attributed to NmC（C型髄膜炎菌）が発生し，1万人近い感染者が発生した。
In 2009 and 2006, meningitis outbreaks caused by _N. meningitidis_ serogroups A (NmA) and X (NmX), respectively も報告されている。
_H. influenzae_ and _S. pneumoniae_ は，ニジェール国内の細菌性髄膜炎による負荷に深く関わる 2 other important pathogens である。
以前から for many years，髄膜炎菌，肺炎球菌，およびインフルエンザ菌に対して認可されたワクチンが入手可能となっている。これらの菌には（serotypes or serogroups と呼ばれる）異なる菌株 different strains が複数あるが，ワクチンは最も被害の大きい株 the most harmful strains に対する感染防御がデザインされている。カバーする菌株や入手は大いに改善されてきたが，no universal vaccine against these infections exists（すべてに有効なワクチンは存在しない）
In the African meningitis belt において，A群結合ワクチン接種キャンペーンと定期接種への導入 the introduction of a meningococcal A conjugate vaccine through mass preventive campaigns (since 2010) and into routine immunization programmes (since 2016) 以前は，A群 meningococcus serogroup A が髄膜炎流行の（原因菌の）80-85% を占めていた。ワクチンを接種した人々において，the incidence of serogroup A meningitis の発生は 99% 以上減少し，2017年以降 serogroup A case は確認されていない。
しかしながら，serogroup B を除く他の血清型菌による感染とアウトブレイク cases of meningitis and outbreaks due to other meningococcal serogroups は，引き続き発生している。
