In mid-2023, 8 autochthonous malaria cases (_Plasmodium vivax_) were identified in Florida and Texas (3); in both states, the autochthonous cases occurred in the vicinity of an imported malaria case.
Investigation and Outcomes
2023年8月6日、生来健康であった the Maryland National Capital Region の住民1名が、7日間続く発熱、倦怠感、筋肉痛のため受診した。発症前の数ヶ月間に自宅付近を毎日歩き、ダニの接着があったことを報告したが、海外旅行、輸血、注射薬物の使用、その他の血液媒介性病原体のリスクは認められなかった。
初期検査で貧血、血小板減少症、高ビリルビン血症、赤血球内寄生虫が認められ、バベシア症またはマラリアが疑われた。入院となり、海外渡航がなく、ダニへの暴露が報告されたことから、バベシア症治療 empiric treatment for presumed babesiosis was initiated [Atovaquone (750 mg twice daily), azithromycin (500 mg twice daily), and doxycycline (100 mg twice daily) for 7 days]. が開始された。
8月9日、薄層血液塗抹標本で熱帯熱マラリア _Plasmodium falciparum_ malaria with 3.2% parasitemia が確認されたが、遠隔診断 Blood smear telediagnosis at CDC では could not conclusively differentiate between malaria and babesia parasites from the images provided. ...

Great to see these sequences out on @GISAID (ID# 18366401 & 18373263) so fast. Similar to the 2 cases in February 2023, both #H5N1 #influenza viruses belong to clade 2.3.2.1c viruses and similar to viruses circulating in the region for ~10 years....
