Non-autochthonous multibacillary leprosy case - Canada
発症の少なくとも30年前からカナダ Alberta, Canada 在住で最近の州外への渡航歴のない83歳男性のハンセン病域外感染例 an unexpected case of non-autochthonous leprosy について報告する。
2024年10月上旬，男性の娘により上半身の膿も痛みもない紅斑 an erythematous rash に気づかれた。およそ8週間前に開始した認知症薬によるものと考え，服薬を中止したがその後も6週間続いたため，皮膚生検目的に皮膚科医を紹介された。
上腕二頭筋近位外側から採取され提出された検体の所見；
within the dermis there were several well-formed granulomas with a sparse associated lymphocytic infiltrate.
Ziehl-Nielsen & Fite stains 染色で多数の桿状微生物が確認され，抗酸菌 mycobacteria が疑われたが，培養検査はオーダーされていなかった。
2024年12月末，8週間後に皮膚病変 an 8-week history of an erythematous plaque skin eruption が，上胸部，腹部，顔面にも拡がり，三次救急医療施設に入院した。手掌と足底には病変は及んでいなかった。発熱，悪寒，消化器症状，体重減少は認められなかった。CRP と好中球優位の白血球数上昇 a raised C-reactive protein (10.4 mg/L) and white cell count (11 700/microL predominantly neutrophils があり，血小板数は正常だった。胸部レントゲン検査で，右上葉の石灰化のある肉芽腫の所見を認めた。この間，血清陰性関節炎 a seronegative arthritis diagnosis の診断で，prednisone and methotrexate が投与されていた。
the Infectious Diseases Services (ID) から、皮膚の再生検と抗酸菌精査 a full work-up for suspected disseminated mycobacterial infection の依頼があり，The skin biopsies では moderate numbers of AFB in the auramine smear が確認された。
結核菌 _M. tuberculosis_ complex を疑い行われた生検検体の TB Genexpert は陰性だった。

症状が進行したため，the local preliminary identification of _M. leprae に基づき a modified "ROM" (rifampin 600 mg, ofloxacin 400 mg, and minocycline 100 mg) regimen を開始した。
この感染が活性化されたイベントもしくは環境が何であるか不明である。過去の曝露からは数十年が経過し，カナダはハンセン病非常在国である。
2025年1月27日のアイルランドの症例に関する ProMED 投稿の症例は今も常在 still endemic する地域から移住した人物であったため，症状についての診断の中で（ハンセン病も）検討されている。
結論として，ハンセン病は長期間の潜伏期間のため診断が困難であること，はるか昔のイベントであっても the significant utility of molecular assays により数日以内に species identification 同定可能であること、を明記する。
