職業上の曝露以外に、動物や患者との接触はなかった。この女性が処理した All the live poultry は、周辺の店のものに限られていた。様々な農場で飼育された家禽が集められる二次卸市場 a secondary wholesale market から家禽は来ていた。患者から採取された気道分泌物の検体について、様々な検査 -- including real-time reverse-transcriptase polymerase chain reaction (RT-PCR) assay, viral isolation, and full-genome sequencing -- が行われた結果、the novel H7N9 virus の感染が確定診断された。感染源調査として、the 2 stalls in the market の環境からの検体 5 environmental samples や 13 samples from the secondary wholesale market が採取され、隣の店の One poultry-cage specimen が、the novel H7N9 virus が陽性となった。さらに、二次卸市場の糞便の検体 1件から mixed avian-influenza viral nucleic acids (such as, subtypes H3, H7, N8, and N9) が、subtype-specific real-time RT-PCR によって検出された。2株のウイルス、患者 (Nanjing/1) および鳥かご poultry-cage specimens obtained in the neighboring stall (Env/Nanjing) の各 1検体、の分離 に成功した(GenBank numbers, KC896771-KC896778 for the case specimen and KC896763-KC896770 for the environmental specimen)。遺伝子の比較 Genome comparison により、2つのウイルスの相同性は from 98.9 to 100 per cent; 98.2 to 100 per cent の相同性を持つアミノ酸に変換される。いずれの isolates の潜在性機能的アミノ酸部位 potentially functional amino acid sites も、ほ乳類またはヒト馴化に関係するヘマグルチニン hemagglutinin レセプター結合部位の置換 substitution T189A and Q226L (H3 numbering) との関連性を有していた。position 627 of BP2 には、K ではなく E が発現していた [?]。しかし Nanjing/1 [のウイルス株] は、position 591 of PB2 に Q ではなく K が示され、 position 627 of PB2 と共同しており、combination of 591K-627E については、 H5N1 ウイルス感染のマウスの 591Q-627E より毒性がより強力であった (Table S1 in the supplementary appendix) [???]。これらの所見から、この新型ウイルスは新興ウイルスであり、少数のアミノ酸の変異によりヒトに馴化する可能性がある。系統発生学的解析では、the M genes of the 2 isolates が、他のウイルスの中でも特に、avian H9N2 viruses 由来の other available human H7N9 strains [?] と比較して、a unique sublineage を形成していた。これは the novel H7N9 virus の起源に多様性があり、家禽内で再集合が起きていた可能性を示唆する。以上より、the novel H7N9 virus の感染は二次卸売市場から小売りの live-animal market に伝播し、患者に感染したと考えられる。これは、live-animal markets とヒトの感染との疫学的関連性を示した、最近発表された研究結果とも一致する。
