イタリアの中でも人気の観光地の 1つであるトスカーナ the Italian region of Tuscany の当局が，a deadly outbreak of the New Delhi superbug の発生があり，病院管理を強化していると明らかにした。2018年11月以降，同地域 the northwestern area of Tuscany 内の広い範囲で，この多剤耐性の致死性細菌の深刻な蔓延が続き，少なくとも 75人が感染したと，local health authority ARS が述べた。at least 31 people in 17 different hospitals since then が同菌による感染症で死亡したと，12日にItalian media が報じている。斜塔で有名なピサ Pisa でも 31人以上の感染が報告されている。
Superbug NDM-1 (New Delhi metallo-beta-lactamase 1) は，2010年にインド首都で発見されて以来，世界中をパニックに陥れており，最後の砦とされるとっておきの抗生物質のカルバペネム carbapenems に対しても抵抗性を示す。欧州保健当局 The European Centre for Disease Prevention and Control は 6月，a "large outbreak" of the New Delhi superbug in Tuscany の発生を受け，緊急リスク評価 a rapid risk assessment を発表したた。この地域が人気の観光地であることを踏まえ，国境を越えた感染拡大のおそれがあると警告している。
[Mod.ML－ blaNDMs (New Delhi metallo-beta-lactamases such as NDM-1, -2, -3, -4, -5, etc.) は，カルバペネム系抗生物質 carbapenem antibiotics や，ほかのほとんどの βラクタム系抗生物質を破壊する，カルバペネマーゼ carbapenemases と呼ばれる一連の酵素のメンバーである。カルバペネムは，βラクタム系抗生物質の種類の 1つで，ertapenem, doripenem, imipenem, and meropenem などがある。Carbapenems は，カルバペネムに感受性のある様々な感染症のほか，extended-spectrum beta-lactamases (ESBLs) or AmpC beta-lactamases を産生する多剤耐性グラム陰性菌感染症の治療の切り札であった。カルバペネム耐性は通常，カルバペネマーゼ産生能の獲得による。このほかの耐性化メカニズムとしては，カルバペネムのペニシリン結合蛋白への親和性減弱，βラクタム抗生物質の排出増加，外膜ポーリン孔の喪失による外膜の透過性低下などが，カルバペネマーゼ産生と協調している場合もある。several distinct types of carbapenemase: KPCs (_Klebsiella pneumoniae_ carbapenemases such as KPC-1, -2, -3, -4, etc.); OXA types (oxacillin-hydrolyzing carbapenemases); NDMs (New Delhi metallo-beta-lactamases such as NDM-1, -2, -3, -4, -5, etc.); IMP; and VIM (Verona integron-encoded metallo-beta-lactamase) が知られている。カルバペネマーゼは，活性部分 their active sites に結合亜鉛イオン bound zinc ions を有する metallo-beta-lactamases と，同じくセリン serine を有する serine carbapenemases に分けることができる。NDM, IMP and VIM は metallo-beta-lactamases に，KPC and OXA は serine beta-lactamases に分類される。Vaborbactam (meropenem との合剤として販売) および avibactam (the 3rd-generation cephalosporin ceftazidime との合剤として販売) は the KPCs を阻害する。Avibactam は，some OXA carbapenemases の一部阻害する。しかしながら，neither avibactam nor vaborbactam も the metallo-carbapenemases の作用を阻害することはできない。Many of the carbapenemases は，プラスミドやその他の可動性遺伝因子 plasmids or other mobile genetic elements 上にエンコードされている。Mobile genetic elements により，同じ種の菌だけでなく，異なる菌にも，薬剤耐性の遺伝情報が拡散される。Mobile genetic elements にはしばしば，(カルバペネム以外の) 他のクラスの抗生物質への耐性を付与する遺伝情報も含まれている carry ため， これらの細菌は唯一毒性の強い，さもなければ次善の抗生物質，すなわち colistin and tigecycline だけに感受性を示すこととなる］
