Winnipeg [Manitoba] Canada の感染症チームが，2020年3月以降，4例の重症バルトネラ菌心内膜炎 cases of severe _Bartonella quintana_ endocarditis のクラスター感染を確認している。_B. quintana_ は，皮膚の開放創からの感染性シラミの糞 the inoculation of infected body lice feces の移入により感染する。住む家や飲み水を持たない，貧困者の病気である。_B. quintana_ は，偏向性の強いグラム陰性細胞内細菌 a fastidious Gram-negative intracellular bacillus で，歴史的には第一次世界大戦中の "trench fever 塹壕熱" に関係していた。都市部では，ホームレスやシラミ感染 pediculosis のある人々の間のアウトブレイクが報告されている。
症例１および２はともに 2020年3月に診断された。3例目と4例目は 7月に診断されている。4例はすべて血清抗IgG抗体価が8192倍を超えていた（バルトネラ心内膜炎患者はほとんどの場合 800倍）。1例目と2例目については，切除された心内膜で細菌の遺伝子検査陽性 16s rRNA confirmation of _B. quintana_ が確認されている。血清検査では，_B. quintana_ と _B. henselae_ （ネコひっかき病の病原体）との間の交差反応が認められるが，症例 ３および 4 には，同 1 および ２との疫学的関連性が認められた一方，ネコとの接触はなく，_B. quintana_ 感染と判断した。いずれの症例にも，大動脈弁と僧帽弁のいずれかまたは両方に大型の疣贅が認められた。3例については，真菌性動脈瘤が確認されている。1例は疣贅塞栓を伴っていた ... 症例 １，２，３ は，市内の同じ援護施設に過去 12か月間滞在していた。また，症例 ３および ４は the same rural remote community を行き来していた。抗生物質による治療で完治し，はじめの 2例については，弁置換術も行われた。
