Overview

3月10日の時点でドイツから12例の，ボツリヌストキシンの胃内注射 intragastric injection of botulism neurotoxin (BoNT) による医原性とみられるボツリヌス症例 cases of clinical botulism, apparently iatrogenic が報告されている。症例はドイツ各地の中年成人で，between [22 and 25 Feb 2023 の期間にトルコにおいて 1000-1500 units of BoNT による治療を受けていた。臨床症状は軽症から重症にわたり，重症例は集中治療室に入院した数例が含まれる。情報が得られた8例についてはすべてイスタンブール Istanbul, Turkey の同じクリニックで治療を受けていた。旅行会社が企画した治療は示唆されていない。患者らは a WhatsApp group で病院と連絡を取っていた。使用された製剤名 the name of the BoNT product を記憶していた合計3例が同じ製剤であったと報告している。

オーストリアは，トルコで intragastric injection of BoNTs を受けた25－44歳の女性1名の症例を2月22日に報告している。ドイツとスイスの症例と同じクリニックを自身で予約し治療を受けたと報告している。この患者はボツリヌス症の症状 botulism symptoms (眼瞼下垂 ptosis, 嚥下障害 dysphagia, 呼吸困難 dyspnea, 頚部脱力 neck weakness, 全身性筋力低下 generalised muscle weakness) のため入院となった。

スイス (information reported to ECDC by France on behalf of Switzerland) が 1 suspected case of botulism (female, 45-64 years) を報告しており，intragastric injection of BoNT, also administered in Turkey on [22 Feb 2023] との関連する可能性があるとしている。ドイツおよびオーストリアの症例と同じクリニックで治療を受けている。

Background information

2019年フランスは，減量目的にエジプトで an intragastric injection of BoNT を受けた，フランス国内で入院となった25-44歳女性のボツリヌス症 a suspected case of botulism を報告している。

2022年8月以降 A WHO medical product alert として，5 falsified batches of a BoNT product detected in 5 countries - ヨルダン Jordan [May 2022], トルコ Turkey [May 2022], クウェート Kuwait [June 2022], 英国 United Kingdom [June 2022], and ポーランド Poland [July 2022] を報告している。しかし，これらのバッチ製品が今回の症例の治療に用いられたかは判っていない。これまでにも，エジプトおよびトルコなどで，様々な医療措置として用いられた counterfeit or unlicensed BoNT が関係する医原性ボツリヌス症が報告されている。

Methods

A randomized, observer-blind, placebo-controlled, dose-selection, phase 1 trial was conducted in 18-49-year-old adults at 9 centers (7 in the United States 米国, 2 in Puerto Rico　プエルトリコ) from 13 Nov 2017 to 24 Nov 2020 ...

Results

TAK-426 at 10-μg had an acceptable safety profile in FV-naive and FV-primed adults up to 24 months after dose 2. Seropositivity for neutralizing antibodies was 100% at 1 year, and 93.8% and 76.2% at 2 years in FV-naive and FV-primed groups, respectively. TAK-426 responses were comparable in magnitude and kinetics with those elicited by natural Zika virus infection.

Conclusions

These results support the further clinical development of TAK-426 for both FV-naive and FV-primed populations.

The Pan American Health Organization (PAHO) updated its Arboviral situation summary のデータによると，a coral island in the Lesser Antilles of the West Indies が2022年に広範囲のジカウイルスアウトブレイク an extensive Zika virus (ZIKV) outbreak に直面していた。ジカウイルス感染伝播はネッタイシマカのいるほとんどの地域で発生する可能性があり，すなわち，throughout much of the Caribbean Islands にそのおそれがあることを意味する。2022年，the PAHO は a total of 37 869 ZIKV cases in the Region of the Americas in 15 out of 52 countries and territories を報告し，その大部分がブラジルからの報告であった。しかし，人口10万人あたりの累計発生数は，人口約28万人のバルバドスが48人で，人口2億人以上のブラジルはわずか16人となる。

先天性ジカ症候群児 Children with congenital Zika syndrome (CZS) は，末梢および中枢神経系 the peripheral and central nervous system (CNS) に深刻なダメージがあり，死亡リスクが大きく増加している。しかし，これらの児について，死亡原因 the sequence of the underlying, intermediate, immediate, and contributing causes of deaths について情報が少ない。

The aims of this study are to describe the sequence of events leading to death of children with CZS up to 36 months of age and their probability of dying from a given cause, 2015 to 2018.
