Global situation summary

the Global Polio Eradication Initiative によると33 countries globally が，異なる型のウイルス感染のある国 states infected with different types of polioviruses に指定され，この中には2023年に野生株1型ウイルス wild poliovirus type 1 (WPV1) の域内感染 endemic transmission が確認されている2か国（アフガニスタンとパキスタン）も含まれている。

4月5日時点で，アフガニスタン，マラウイ，モザンビーク，パキスタンの4か国で WPV1 の感染が発生していると考えられている。circulating vaccine-derived poliovirus type 1 (cVDPV1) が発生しているのは，マダガスカル，モザンビーク，マラウイ，コンゴ民主共和国の4か国，circulating vaccine-derived poliovirus type 3 (cVDPV3) はイスラエルで報告され，29 countries with circulating vaccine-derived poliovirus type 2 (cVDPV2) となっている。

Situation summary in the Region of the Americas（南北アメリカ）

2023年3月21日にペルーの当局 the National Focal Point (NFP) for the International Health Regulations (IHR) of Peru が PAHO/WHO に対し，a confirmed case of vaccine-derived poliovirus type 1 (VDPV 1) を報告した。発症時に生後14か月であったこの症例は，部族コミュニティ an indigenous community in the district of Manseriche in the Datem del Marañón province of the Loreto department の住民で，ワクチン接種および渡航歴はなかった。2022年12月27日に持続する発熱のため地域保健センターを受診し，同28日に the Loreto Regional Hospital 病院に紹介となり，同29日に下肢の麻痺を発症し，便検体が採取され検査機関 the regional reference laboratory に送付された。免疫不全は除外されている。ブラジルの地域紹介ラボ the regional reference laboratory for polio, the Oswaldo Cruz-Fiocruz Foundation in Brazil において，vaccine-derived poliovirus serotype 1 (VDPV type 1) by real-time PCR と確認された ... 全遺伝子塩基配列解析の結果，31 nucleotides difference compared to Sabin 1 virus (VP1) が示され，現在の流行国の1型ポリオウイルスのいずれとも異なっていた genetically related to any other VDPV1 previously sequenced, including those currently circulating in countries with cVDPV1 outbreaks ため，a new VDPV1 とされた。その後の調査で，4例の親族の無症候性感染が確認された asymptomatic contacts of the confirmed case were identified among his relatives 。また the town of Atahualpa において生後18か月の急性弛緩性麻痺症例 a case of acute flaccid paralysis (AFP) が確認され，現在確定診断が行われている。過去4年間のペルー国内のポリオワクチン接種状況は < 95% で，2020 (71.58%) and 2021 (78.77%) は80％未満だった。全国の45％の地域で接種率が80%を下回っていた ...
