Update on iatrogenic botulism cases in Europe

2023年2月下旬から3月30日までの間に，87例の医療行為としてのボツリヌストキシン botulinum neurotoxin (BoNT) 注射に関係するボツリヌス症 cases of botulism linked to intragastric injection が，ドイツ Germany (30), オーストリア Austria (1), フランス France (1), スイス Switzerland (2), and トルコ Türkiye (53) から報告された。
3月10日の最新報告 the last ECDC update 以降，18 cases in Germany, and 1 new case each in France and Switzerland の増加があった。現時点で入手可能な情報によると，すべての患者に減量を目的とする医療的介入があった。その処置は2023年2月3日から3月1日の期間にトルコ国内で実施されていた。患者の報告によると，肥満治療として胃（筋層）内へのボツリヌストキシン注射 intragastric botulinum neurotoxin (BoNT) injections を2か所の私立病院において受けていた: one in Istanbul and another in Izmir。ドイツの患者についてドイツ政府当局は，注射されたドースが between 1000 and 2500 units of BoNT であったと報告している。症状は軽症から重症例に及んでおり，重症例は入院となり，入院患者のうちの多数の症例 a number が集中治療室で抗毒素 botulinum anti-toxin 治療を受けていた。相当する期間に intragastric BoNT treatment のために Istanbul and Izmir に渡航した旅行者は，特に筋力低下，呼吸・嚥下困難の症状がある場合，医療機関を受診することを強く勧める。EU/EEA citizens に対し，トルコ国内で減量目的の胃内 BoNT 治療を受けないことを推奨する。トルコ政府当局による調査で，治療に用いられた licensed BoNT products は適応外使用 administered off-label であったと明らかにされている。関連する両施設ともに同治療を中止している。

- アイルランド Ireland: On 22 Mar 2023, the Irish Health Protection Surveillance Centre (HPSC) ... , reporting 10 confirmed iGAS cases for the period 12 to 18 Mar 2023. Among these, most were in the age groups 0-4 years (2) and 55-64 years (2).
In 2023, as of 18 Mar 2023, a total of 117 iGAS cases had been notified in Ireland, compared to 8 cases reported in the same period the year before.
Since October 2022, 6 paediatric deaths（小児死亡例）from iGAS have been reported, and 10 deaths in adults（成人死亡例）.
- ギリシャ Greece: the Greek National Public Health Organization (NPHO) was recently notified of another fatality（死亡例）related to iGAS in an 18 month old child. In 2023, 3 deaths linked to iGAS have been reported in Greece.
Summary
2022年12月2日，the European Union/European Economic Area [EU/EEA] and the United Kingdom (UK) において，侵襲性感染と猩紅熱 iGAS and scarlet fever notifications caused by diverse emm types の増加が報告された。増加を報告したのは，フランス，アイルランド，オランダ，デンマークで，ほかの国でも昨シーズンからの増加が報告されたが，パンデミック前に比べて発生数は少なかった。
最も感染が多い年齢層は10歳未満と65歳超で，受診者数のピークはクリスマス前の2022年12月であり，1月には減少に転じた。

炭疽 Anthrax は最も重要な人獣共通感染症の1つで，基本的に草食動物の感染症であるが，しばしばヒトにも感染する。病原体の炭疽菌 _Bacillus anthracis_ は，グラム陽性，好気性，芽胞形成性，非動性，桿菌である。芽胞は環境条件に耐性があり，土壌中 contaminated soil で長期間生存し，野生動物や家畜の感染源 the main reservoir となっている。現在も低所得国で発生があり，疾病負担と経済的困難の原因となっている。ヒトは病気または死亡した動物，汚染のある動物食品（製品）との接触や，芽胞との接触により感染する。従来から3つの病型，皮膚炭疽，消化管炭疽，吸入炭疽，があり，いずれにも敗血症や髄膜炎を発症するおそれがある。最近，薬物常用者の "injectional anthrax" と呼ばれる致死率の高い（>33%）新たな病型が報告されている。初期症状は他の疾患と類似するため，細菌培養または分子学的検査により菌を同定することが重要である。治療に関しては，ヒトからの分離菌は一般的にほとんどの抗生物質に感受性があり，penicillin G and amoxicillin が第1選択となり，代替薬は ciprofloxacin and doxycycline になる。全身性感染 systemic anthrax に対し，1または複数の抗生物質を併用することが示されている suggested 。ヒトの感染対策は，有効な動物の感染コントロールに依存する。獣医学的には芽胞ワクチン Spore vaccines が使用されており，ヒトに対しては無細胞ワクチン an acellular vaccine の使用が可能であるが，利用は限られている。
