薬剤超耐性サルモネラ感染症 Extensively drug-resistant (XDR) _Salmonella_ が初めて報告されたのは、2016年のパキスタン・シンド州 Sindh, Pakistan, で、それ以降 several districts から multiple XDR _S. enterica_ serovar Typhi variants belonging to clade H58 and carrying multiple novel genomic integrations of the extended-spectrum beta-lactamase gene が報告されてきた。症例の多くは meropenem and azithromycin で治療され、national guidelines でもパキスタンの感染例 cases of _Salmonella_ Typhi in Pakistan の治療に同薬剤が推奨されている。
We report a case of carbapenem-resistant _Salmonella_ Typhi infection that required treatment with a last-resort antimicrobial drug.
Peshawar, Pakistan の7歳女児が2022年7月に発熱、悪寒、排尿症状のため、政府系病院を受診した。中等度の心房中隔欠損 moderate atrial septal defect, 、大きな入口部心室中隔欠損 large inlet ventricular septal defect、小さな動脈管開存 small patent ductus arteriosis defects, 心室肥大 ventricular hypertrophy, 重度肺高血圧症 severe pulmonary hypertension、を含む先天性心疾患のあるダウン症候群 Down syndrome 児で、入院して抗生物質治療を必要とする下気道感染症を繰り返していた。以前に、Macrolides and carbapenems の処方を受けていた。定期予防接種を受けていたが、腸チフスワクチンは受けていなかった...
腸チフスの疑いで入院となり、入院時の白血球数は7600、C-reactive protein (CRP) level は120 mg/dL であった。初期治療 Initial empiric treatment としてセフトリアキソン ceftriaxone が投与されたが、血液培養の結果、XDR _Salmonella_ Typhi は、ampicillin, 3rd-generation cephalosporins, fluoroquinolones, chloramphenicol, and cotrimoxazole (trimethoprim/sulfamethoxazole) に耐性があるが、susceptible to azithromycin and meropenem が示唆されたため、経静脈的に intravenous meropenem (300 mg 3 x/d) 、経口で oral azithromycin (200 mg/5 mL 1 x/d) for 10 days の治療に切り替えられた。
The patient responded positively to treatment, indicated by decreased CRP levels (7 mg/dL at treatment completion). No clearance blood cultures were obtained before discharge.
1ヶ月後の8月、発熱と食欲低下があり、私立検査機関で採取された血液の培養の依頼があった。the blood culture on a BACT/ALERT VIRTUO automated system (bioMerieux, ). ...We confirmed results by using bioMerieux Etest strips. MIC for colistin, determined by disk-elution method, was 1 microgram/mL.
After admission, treatment with intravenous meropenem (700 mg 3 x/d infused over 2 h) and intravenous colistin (40 mg 2 x/d) was initiated. The patient was closely monitored and became afebrile on day 3 of therapy. A 2-dimensional echocardiogram showed no vegetations. Results of repeat blood cultures and fecal cultures were negative. The patient was discharged after completing 11 days of treatment. At a 2-week follow-up visit, she was back to her usual state of health; typhoid vaccination 2 weeks later was recommended....
