2023年のバングラデシュ国内で、ニパウイルス the Nipah virus により10人が死亡し、過去7年間で最多の死者数を記録している。このほかにも、14人の同ウイルス感染が確認されている。2023年はじめての感染が報告されたのは Narsingdi [district] recorded であったと、the Institute of Epidemiology, Disease Control, and Research (IEDCR) から報告された。
12月10日に the IEDCR headquarters で開催された会議 a discussion titled "Spread and Risks of the Nipah Virus" において、同ウイルスが母乳中で確認されたことが初めて報告された。
冬期の祭事で date juice が振る舞われ、この恐ろしい病気が全国に拡がることにつながっているとして、専門家らは摂取しないようアドバイスを行っている。コウモリの分泌物や唾液からニパウイルスの感染が拡がるため、ウイルスに汚染された生の raw date palm sap を摂取してもウイルスに感染するおそれがある。患者から、家族や医療従事者がウイルスに感染する可能性もあると説明されている。
"safe" and "boiled" などのラベルが付けられオンラインで販売されている Date juice によって、全国的にウイルス感染が拡大している。
IEDCR data によれば国内初の感染例は the 1st Nipah patient in Bangladesh was identified in Meherpur in 2001 で、これまでに 7 districts of the country で発生が確認されている。以後、全国でこれまでに累計339例のニパウイルス感染例が確定診断され、うち240例が死亡し、致死率は71.44% [in fact, 70.8%] となっている。過去、2015年に15人が感染し11人が死亡し、その後は年間2から8例の感染が続いていた...

2023年11月21日、3週間のタイ旅行から帰国した、急性顕性ジカウイルス感染症 acute symptomatic Zika virus の患者が2名、当院を受診した。
34歳の女性患者は、11月15日から頭痛、関節痛、疲労感、有痛性リンパ節腫脹、盗汗の症状があり、同17日に発疹が現れた。
この女性の33歳のパートナーにもその1-2日前から同様に、頭痛、四肢痛、盗汗があり、同16日に発疹の症状が現れた。
いずれの患者も、発熱は認めなかった。
患者らは受診時、四肢痛、痒疹、倦怠感、有痛性リンパ節腫脹を訴え、女性には手掌の紅斑と、全身性の極めて柔らかく境界不明瞭な腫大したリンパ節 （?generalized strikingly tender borderline enlarged lymph nodes) が確認された。男性の躯幹の発疹はわずかだった presented with a slight exanthema on his trunk。
両者の血液検査では CRP がやや上昇し、女性は LDH がわずかに上昇していたのみであった。
血清学的検査でジカウイルス抗体の高値と陽転 increasing high antibody titers in IFAT (IgG up to 1:2560 and 1:10240, respectively, 1 week later) and seroconversion in the IB が確認され、尿検体では Zika virus RT-PCR from urine samples were both positive だった。
患者らはいずれもタイ南部を旅行し、発症までの潜伏期間中（3-14日）に、from Krabi via Khao Sok and Koh Phangan to Koh Samui を訪れていた。
Koh Phangan (4-6 and 8-10 days before onset) で最も多く蚊族の刺咬を受けていたことを記憶している。いずれの患者も問題なく回復し、妊娠はなかった; pregnancy was not an issue.
Relevance:
最近、大規模なアウトブレイク significant Zika virus outbreaks は報告されていないものの、タイを含む常在地域への旅行者の現地での感染リスクは依然として存在する。医療関係者らは、該当地域でのアウトブレイク発生への注意を怠らず、妊娠可能年齢の旅行者への周知を高める必要がある。
