横浜刑務所で２８５人が食中毒、ウエルシュ菌検出、いずれも軽症
横浜市は２５日、同市港南区の横浜刑務所で食事をした１０～８０代の受刑者ら２８５人が下痢や腹痛などの症状を訴え、うち６人の便からウエルシュ菌を検出したと発表した。いずれも軽症で、入院者はいないという。
市保健所は、刑務所内で調理した２０日の昼食か夕食に菌が入っていた可能性があるとみて、所内の調理工場を給食の調理業務の禁止処分とし、詳しい原因を調べている。受刑者が作業の一環として調理していた。
市によると、２１日に刑務所から「２０日の夜から体調不良者が１００人以上いる」と連絡があった。２０日は昼食にチキンカレーや大根サラダなど、夕食に担々ごま鍋、カボチャと小倉あんの煮物などが提供されたという。
[Mod.LL－ 産生されたトキシンを嚥下するブドウ球菌性食中毒 staphylococcal foodborne illness と違い、ウェルシュ菌感染症 _C. perfringens_ disease においては体内の消化管内で the enterotoxin が産生される。

"1. Name of the organism
_Clostridium perfringens_ は、嫌気性グラム陽性芽胞形成桿菌 an anaerobic, Gram-positive, spore-forming rod で、環境中およびヒトや動物に広く存在する。芽胞は、土壌やヒトや動物の糞による汚染のある場所などに存在する
"2. Nature of acute disease
_C. perfringens_. による食中毒には一般的に Perfringens food poisoning の用語が使われ、type C strains が原因のより重篤だがまれな疾患もあり、壊死性腸炎 enteritis necroticans, or pig-bel disease と呼ばれている。
"3. Nature of disease
一般的な症状は、強い腹痛 intense abdominal cramps と下痢で、中毒性トキシンを産生するのに十分な多量の菌で汚染された食品の摂取の、8-22時間後から始まる。通常は24時間以内に治まるが、一部の患者では強い症状が1－2週間続くことがある。脱水、その他の合併症による少数の死亡例も報告されている。

"Infective dose: The symptoms are caused by ingestion of large numbers (greater than 100 million＝1億以上) vegetative cells.
消化管内（もしくは試験管内）でのトキシンの産生には胞子形成 sporulation が関与する。本疾患は a food infection であり; これまでに中毒 intoxication (that is, disease from preformed toxin すでに産生されていたトキシンによる疾患) の可能性が示唆されたのは1件のみ only one episode にとどまっている。
"4. Diagnosis of human illness
診断は臨床診断と、遅い発症時期からなされる。確定診断は患者の便中のトキシンの検出による。疑われる食品や患者の便中の異常な数の起因菌の同定による、細菌学的診断も可能である。
"5. Associated foods
多くの場合、調理された食品の温度管理に問題があることが原因で、調理されたあとに存在する少量の菌が、食品が冷め保管されている間に増殖する。原因と考えられる食品として、肉、肉製品、グレービーソースが最も多い。
"6. Relative frequency of disease
米国内では1162 cases in 1981, in 28 separate outbreaks が発生し、この20年間少なくとも毎年 At least 10-20 outbreaks が発生している。数十人から数百人が感染する。ウェルシュ菌検査が必ずしも行われるわけではないため、報告されるアウトブレイク数は実数よりも少ない。
CDC estimates that about 10 000 actual cases occur annually in the USA.
"7. Course of disease and complications
通常、24時間症状が持続し、高齢者や弱者では1－2週間続くこともある。合併症や死亡はきわめてまれ。
"8. Target populations
学校のカフェテリア、病院、高齢者施設、刑務所などの施設内給食は、配食の数時間前にたくさんの食事を作るため、perfringens poisoning が起こりやすい。the case of pig-bel syndrome 以外で30歳未満の患者に合併症がおこることはほとんどない。高齢者ではより重症化し症状が長引きやすい。
"9. Food analysis
Standard bacteriological culturing procedures are used to detect the organism in implicated foods and in feces of patients. Serological assays are used for detecting enterotoxin in the feces of patients and for testing the ability of strains to produce toxin. The procedures take 1-3 days（1－3日かかる）." (FDA)]

Summary
類鼻疽 Melioidosis, caused by _Burkholderia pseudomallei は、まれであるが致死性の細菌感染症で、熱帯及び亜熱帯地域に常在する。典型的には汚染された土壌または淡水との接触により感染する。今回の調査以前、米国本土 the continental United States の環境中で _B. pseudomallei_ が検出されたことはなかった。
今回われわれは、この3年間に経験した、ミシシッピ州の同じ沿岸地域 the same Mississippi Gulf Coast county 在住の3例の patients with melioidosis within a 3-year period について報告する。患者は同じ菌株 the same Western Hemisphere _B. pseudomallei_ strain に感染しており、患者のうちの1人の自宅敷地内で採取された3 environmental samples で検出された菌株だった。
These findings indicate local acquisition of melioidosis from the environment in the Mississippi Gulf Coast region.

マーモセット white-tufted marmosets (_Callithrix jacchus_) で確認された変異株に近い狂犬病ウイルス Rabies virus variants が北東部のコウモリ bats in Ceará state, northeast Brazil で確認された。
FAPESP and led by researchers at the Federal University of São Paulo's Medical School (EPM-UNIFESP) による研究で the Journal of Medical Virology に掲載される。
マーモセットとコウモリの変異株の類似性に加え、感染伝播の重要な役目を果たしている吸血コウモリの fruit- and insect-eating bat species が保有していたことが、研究者に衝撃を与えた。
"7頭の marmosets が最近狂犬病検査で陽性となった。襲われた患者が狂犬病で死亡しており Ceará 州に狂犬病は常在しする。そのような死亡例が2023年5月に1例発生している" と説明された。
near a town called Cariús 近郊の農場で働いていたこの36歳男性は、2月に襲われた後に死亡した。4月に受診した時に症状は始まっており、まもなく死亡した。

Bluetongue has been detected in a 3.5-year-old dog（3歳半のイヌ）.
The animal lived on a Dutch dairy farm with both cattle and sheep （ウシやヒツジのいるオランダの酪農場）。
Research by Wageningen Bioveterinary Research (WBVR, part of Wageningen University & Research) showed that the animal was infected with bluetongue virus type BTV-3/NET2023, the same virus currently found in sheep and cattle in the Netherlands.
