Drug-resistant _Trichophyton_ fungus represents emerging threat in USA
11例の白癬菌感染例 _Trichophyton indotineae_ infections in New York City from May 2022 to May 2023 について、a new JAMA Dermatology report で報告された。この真菌は新興した公衆衛生上の脅威と捉えられており、抗真菌治療の第1選択薬である terbinafine, a 1st-line oral antifungal にしばしば不応性の、広範囲の真菌感染をおこす。
_T. indotineae_ は、躯幹、四肢、そけいなど広い範囲の皮膚病変 an extensive rash composed of plaques を生じる。
近年、南アジアで複数のアウトブレイクが報告されており、標準の経静脈及び経口の抗真菌治療に反応しない皮膚病変を伴っている。米国を含む世界中で患者が確認されている。米国内ではじめての症例は2023年5月に2例が確認されているが、後方視的検討では2017年から米国内で分離されていることが確認された。
今回の報告で著者らは、6 men and 5 women with a median age of 39 の感染について報告している。2人が妊娠女性で、1人が未診断であったリンパ腫に罹患していた。皮膚病変と掻痒を発症するまでの2年間について、11人中9人がバングラデシュへの渡航を報告している。3例は家庭内感染伝播により同菌に感染したとみられ、1人は渡航歴も患者との接触も認められなかった。すべての患者が相当な診断の遅れを経験しており、3から42ヶ月までの平均10ヶ月間を要している。8例がステロイドを処方されていた。すべての患者がすくなくとも1回は外用抗真菌薬を処方されていたが、monotherapy で有効だったものはなかった。
治療に当たった皮膚科医らは、多くの場合この菌について知識がなかった（気づいていなかった）often unaware of _T. indotineae_ diagnosis 。
無効でなかったのは唯一 Only itraconazole therapy did not fail だったが、治癒まで相当長い期間が必要だった...

国内で 2例の日本脳炎 confirmed cases of Japanese encephalitis が確認されたと、15日 The Department of Disease Control and Prevention が発表した。患者らは高雄市 Kaohsiung City の70代女性と60代男性だった。男性は発症から9日目に死亡し、2024年初の日本脳炎による死亡例となった。
Case 1 is a woman in her 70s from Zuoying District, Kaohsiung City.
6日に発熱、咽頭痛、嘔吐、めまいで受診し、7日に再診となった。救急室において、振せん、発語遅延, slowed speech の症状がみられ、入院となった。
Case 2 is a man in his 60s from Gangshan District, Kaohsiung City. H
5月4日に腹痛のため入院し、同6日に発熱し、9日には項部硬直、顔面神経麻痺、嚥下障害、聴覚障害などの症状を発症し、意識状態の変化がみられた。状態の悪化により、5月9日に死亡した。

Cases of invasive meningococcal disease (IMD) reported in travelers returning from the Kingdom of Saudi Arabia

髄膜炎菌 _Neisseria meningitidis_ により髄膜炎菌性疾患 meningococcal disease が発生し，髄膜炎菌血症を伴う髄膜炎と伴わない髄膜炎 meningitis with or without meningococcemia がある。髄膜炎のない髄膜炎菌血症も起こりうる。
（全人類の）およそ10人に1人は，発症することなく鼻咽頭内 in their nasopharynx に同菌を保有し，キャリア状態 being "a carrier." と呼ばれている。ときにこの菌が体内に侵入し，何らかの疾患を発症させ，髄膜炎菌性疾患 meningococcal disease と呼ばれる。
世界中で発生する疾患の多くが，5種類の血清グループ [型・群] Five of the serogroups (types) of _Neisseria meningitidis_ -- A, B, C, W (also called W135), and Y -- によるもので，このうち3種類 serogroups (B, C, and Y) が米国内の疾患の多くの原因となっている。Serogroup A がアフリカの流行ベルトにおける古典的タイプ classical type seen in the epidemic belts in Africa but others are now occurring であったが，現在は他のタイプによるものも発生している。
