Introduction

製氷機 ice machines はいくつもの機械部品から構成され、微生物の増殖やバイオフィルムの形成がおこりやすい。医療行為の中で用いられる氷や水は、水を介する日和見感染の病原体による医療関連感染症の感染経路であることが知られており、その中にバークホルデリア属 _Burkholderia_ spp も含まれる。_Burkholderia multivorans_ は a member of the _Burkholderia cepacia_ complex で、土壌中や水中に偏在する、水に関連するグラム陰性日和見感染菌である。

この微生物によるアウトブレイクは、医薬品の汚染 contaminated medications, 液体医用製剤 aqueous medical products, 医療機器 medical devices に関係し、抗生物質に耐性であることから重要度が高く、特に免疫に問題のある患者や重症患者では重篤な感染症となり得る。_B. multivorans_ は嚢胞性線維症 cystic fibrosis の患者の喀痰で確認されることが多いが、そのほかに分離されることはあまりない。

During August 2021-July 2022、カリフォルニア州南部の2か所の病院 hospitals in a health care system in southern California (hospitals B and C) から _B. multivorans_ infections in 23人の患者 patients without cystic fibrosis の感染が報告された。

アウトブレイク調査で、製氷機の氷と水がケアの目的 clinical care activities (e.g., ベッドバス bed baths, 嚥下機能評価 swallow evaluations, and 鎮痛用局所冷却パック topical application of ice packs for analgesia) に使用されていたことが分かった。

患者から分離された菌と同じ遺伝子配列の菌 The same sequence type (ST659) of _B. multivorans_ identified among the patients が hospital B ice machines の氷と水から分離された。この型の菌 This sequence type of _B. multivorans_ はこれまで米国内で確認されたことはなかった。cystic fibrosis のない患者の _B. multivorans_ infections 感染が、他の病院 other hospitals in California and Colorado からも報告されたことから CDC and state and local jurisdictions はさらに調査 additional investigations を進めた。

5例めのデング熱・地域内感染例 a 5th locally acquired case of the mosquito-borne illness dengue について、ロサンゼルス郡保健当局 Los Angeles County health officials は3日現在調査を継続しており、今回は a resident of El Monte の住民の感染例である。

最新のこの患者には、これまでの4例の感染例との関連が確認されていないと、the county Department of Public Health が2日述べた。同郡内ではこれまでに3例の小集積 a cluster of 3 infections in the Baldwin Park area と先週の a 4th case in a Panorama City resident が確認されていた。当局は the spate of local cases "unprecedented."としている。地域内感染の患者の中で、デング熱常在地域への渡航歴のあるものはいなかった。

2023年10月と11月にそれぞれ、Pasadena and Long Beach において、地域内感染が1例 single locally acquired cases of dengue ずつ保健当局により確認された。

[Mod.TY－デングウイルス感染伝播に関係したベクター蚊族は示されていないが，過去の投稿のコメントでは，ネッタイシマカ _Aedes aegypti_ or ヒトスジシマカ _Aedes albopictus_ のいずれかの可能性があり，いずれも20年以上前から California 州で確認されている。

Note that both _Aedes aegypti_ and _Aedes albopictus_ have been found in Los Angeles County. ]
