小児科学会 the Malaysian Paediatric Association (MPA) は，危険性のある年齢層 a susceptible age group へのデング熱ワクチンの使用を認めていないので，ワクチン接種によって起きる重症のデング熱症状について心配する必要はないと，説明している。世界初の Sanofi Pasteur 社製のワクチンの接種は，9歳から 45歳だけに認められており，ワクチン接種により重い症状が起きる可能性がある，より年齢が小さい子どもへの接種は認められていないと述べた。数年前にデング熱の専門家が 2回のデング熱感染のある患者で報告したような症状は，ワクチン接種者には見られないとしている。初めて抗体依存性増強現象 the antibody-dependent enhancement (ADE) phenomenon を報告した研究者は，4種類あるデングウイルスのうちの 1種類に感染すると，2種類目のデングウイルス感染により，出血熱などの重い症状が引き起こされる可能性があると報告した。この ADE theory によると，異なる種類のデングウイルスの感染の間隔が長ければ長いほど，より重症化すると言うものだった。ADE はマウスの実験で観察された後，キューバにおいて，それほど重症ではなかった 1型ウイルスによるアウトブレイクの 10-20年後，2型デングウイルスによる，より重症のアウトブレイクが発生したことにより確かめられた。同様の現象は，デングウイルスに感染した母親から生まれた乳児でも観察され，児が感染すると重症化する確率が著しく高くなった。デング常在 10か国の 3万5千人以上での，第Ⅲ相臨床試験データ Sanofi Pasteur's data from phase 3 clinical trials of the vaccine - during year 3 of the trial - では，ワクチンを接種された 5歳以下の小児は，非接種群に比べデング熱による入院率が高かった。データでは， 54.2 % の児が，治験参加時の血清学的検査で陰性だった。同社は，ADE phenomenon に対する直接的な言及は避けており，検査陰性児の入院リスク上昇の理由の明確な説明もしていない。「9歳以上への接種に限られる」と説明するに留まっている同社に対し，年齢は being dengue naive (not exposed to dengue previously)ことの言い換えに過ぎず，はっきりと ADE in phase 4 studies について説明する必要があると，指摘されている ...

急性デング熱感染 DENV-4 infection 6日目の患者が，錯乱と失語 new acute neurological symptoms of confusion and expressive aphasia を発症した。脳脊髄液検査で，リンパ球性細胞増多と抗体陽性 lymphocytic pleocytosis with a positive CSF dengue IgM and IgG を認めた。これ以外の髄液検査項目は陰性だった。他のフラビウイルスが常在する地域への渡航歴はない。DENV-4 により脳炎を発症した可能性がある症例が報告されており，多臓器の障害とデング出血熱により死亡している。今回の症例は，血漿漏出，出血，臓器障害等の，他の症状は認められなかった。初めての，完全回復したデング熱脳炎症例 the first known case of DENV-4 encephalitis である。

急性デング熱感染 DENV-4 infection 6日目の患者が，錯乱と失語 new acute neurological symptoms of confusion and expressive aphasia を発症した。脳脊髄液検査で，リンパ球性細胞増多と抗体陽性 lymphocytic pleocytosis with a positive CSF dengue IgM and IgG を認めた。これ以外の髄液検査項目は陰性だった。他のフラビウイルスが常在する地域への渡航歴はない。DENV-4 により脳炎を発症した可能性がある症例が報告されており，多臓器の障害とデング出血熱により死亡している。今回の症例は，血漿漏出，出血，臓器障害等の，他の症状は認められなかった。初めての，完全回復したデング熱脳炎症例 the first known case of DENV-4 encephalitis である。
