アマゾン盆地 the Amazon basin のジャングルツアーを含む南米への旅行から帰国した 60歳男性の，Mayoro ウイルス病 Mayaro virus (MAYV) が確認されている。カナダ国内で初めての症例 the 1st confirmed Canadian case ではあるが，同地域への旅行者や住民の感染については複数の報告がある。
2018年2月28日にリマ Lima, Peru を出発し，12日間にわたって同国内各地を回った。3月12日からの Jungle tour の起点は Puerto Maldonado, Peru で，the Amazon basin での 4日間では，jungle hikes and boat trips を行った。蚊族による刺咬を報告しており，前腕の刺咬部位は特に痛みがあった。3月18日に空路 Alberta, Canada に戻った。翌日，悪寒戦慄 rigors and chills が始まったが，熱感はなかった。それからの数日間に，膝，肘，足首などの大関節や，手の小関節の痛みが進行し，同時にふくらはぎや四頭筋の筋肉痛と，激しい疲労感を感じた。発症した翌日に家庭医を，4日目に三次病院の感染症専門医を，それぞれ受診し，結膜炎や発疹，咽頭炎などを指摘された。マラリアの予防内服は行っておらず，生ものや生ミルクは摂取していなかったが，帰国後に2日間の下痢があり回復していた。妻も同行していたが症状はなかった。白血球減少 3.7 x10^9/L 以外の血液検査の異常は認められていない。マラリアの厚層・薄層塗抹標本の検査は陰性だった。CRP は 39.2 mg/L (reference range less than 8 mg/L) と高値だったが，4週間後に基準値よりやや高めに低下した。軽度の血尿とタンパク尿が認められ，肝酵素の上昇はなかった。4日目に診察した感染症専門医は，アルボウイルス arboviral 疾患の可能性が高いと判断したが，水との接触がありレプトスピラ症 leptospirosis の可能性もあるため ドキシサイクリン doxycycline が処方された。血液検査で，アルボウイルス，レプトスピラなど発疹性感染症を調べ，鼻咽頭，眼のスワブを採取し，呼吸器疾患ウイルス，結膜炎疾患ウイルス，化膿性連鎖球菌 _Streptococcus pyogenes_の検査も行ったがすべて陰性だった。デング，チクングニア，ジカも陰性であったため，MAYV を含む a related alphavirus が疑われた ... 98 percent identity with a human MAYV genotype D isolate from Peru in 2000 が確認。2005年にも，同地域における this genotype によるヒトの感染例が報告されている ... Mayaro virus and CHIKV は非常に近い関係にあるアルファウイルス closely related alphaviruses であるが，チクングニア感染が南米とカリブ海を含む広い地域で発生しているのに対し，MAYV は南米とカリブ海地域に限局している ... 発熱と発疹のある帰国後の患者らに対して，DENV, CHIKV, and ZIKV を含む serologic panels が用いられることが多い。the alphavirus gel-based molecular assay によって MAYV と鑑別診断されなければ，急性チクングニア感染例 an acute CHIKV infection と診断されていたであろう。
臨床上 the signs and symptoms of CHIKV and MAYV infections are fairly similar である。デング熱やジカウイルス感染では，発疹を伴う急性発熱性疾患で，大小関節痛を伴うことはより少ないが報告されている。したがって，これら 4疾患を鑑別できる明らかな臨床症状はない。
結論として，the laboratory confirmation of a MAYV infection は，the local availability of advanced molecular assays により診断されたが，一般的ではなく often not available in most diagnostic laboratories ，同時に，疫学情報と同疾患を疑うことが必要である。
