新たな（腸管出血性）大腸菌感染症アウトブレイク A new _E. coli_ O157:H7 outbreak が食品医薬品局のリスト the FDA's CORE Outbreak Investigation Table に掲載された。6 other outbreaks についても現在調査が行われている。7件のアウトブレイクのうち4件については、いずれの食品との関連も確認されていない。

今回のアウトブレイク The new _E. coli_ O157:H7 outbreak では少なくとも11人が発症した。患者の居住地、年齢、入院の有無などは公表されていない。

Abstract

Background: Neurological involvement in schistosomiasis（住血吸虫症の神経症状）は深刻な合併症である。早期には治療可能である一方、まれな病態が診断と治療に遅れにつながることも多い。今回、an endemic area における spinal neuroschistosomiasis (NS、脊髄神経住血吸虫症) の臨床症状について報告する。

Methods: A retrospective cross-sectional review was conducted at Althora General Hospital in Ibb, Yemen, from January 2016 to January 2021. ...

Results: 10例 cases of spinal NS with a mean age of 10.1± 3.2 years が特定され、大部分 (90%) が男児で農村部 rural areas の患者であり、全症例に schistosomiasis の感染リスクとされる淡水への暴露があった。発症から治療開始まで平均 33.4± 45.6 days (7-150 days) かかっている。

すべての症例に共通する症状 Common symptoms observed in all patients は、膀胱の機能不全と知覚異常 bladder dysfunction and paresthesia (100%) だった。消化機能不全 Intestinal dysfunction は prevalent in 90% of cases に、また80% に四肢筋力低下または歩行困難 limb weakness or inability to walk の症状が認められた。

確定診断は髄液検査 confirmed through cerebrospinal fluid (CSF) serology in 80% of cases と、便および尿検査 stool and urine exams yielded positive results in 90% and 30% of cases, respectively により行われた。Magnetic Resonance Imaging findings で、髄質病変 medullary lesions in 50% of cases, 馬尾病変 cauda equina lesions in 20%, 多発性病変 multiple lesions in 30% が判明した。

すべての患児に初期治療として経口薬 oral praziquantel と高用量ステロイド high-dose steroids for at least 3 days が処方された。

フォローアップ期間中 During the average follow-up period of 5.6±1.7 months、1名が両側下肢麻痺 lower extremity paraplegia、2 cases (20%) が尿・便失禁 partial improvement with residual deficits including urinary and fecal incontinence を生じた。完治した Complete resolution of symptoms のは7 cases (70%）だった。

Conclusion: Schistosomiasis should be considered in pediatric patients with myeloradicular manifestations（脊髄根症状）, especially in endemic areas. Early identification can be achieved through history, prompt imaging, and CSF serology. In the absence of immediate test results, expert-guided presumptive therapy should be considered to minimize neurological complications.

今回の新たな研究ではほかにも、この比較的まれな病態の成人患者は、COVID-19 感染の14日以上経過後に多く見られ、入院中に胃腸炎症状や胸痛がみられることが多いが、持病のある人は多くなかったと報告されている。のちに MIS-A と診断される患者では、COVID-19 に咳嗽や呼吸困難などの典型的な症状はあまり見られなかったとも述べられている。

between 1 Mar 2020, and 31 Dec 2021 の期間に SARS-CoV-2-associated illnesses により入院となった1万223人の患者の医療記録を解析した結果、53人の people with MIS-A を特定し、106 patients with non-MIS-A COVID-19 hospitalized with acute symptomatic illness と比較した。

MIS-A patients の年齢の中央値は30歳で、ほとんどすべて (90.6%) の患者が重症心臓疾患 severe cardiac illness を発症し、most experiencing gastrointestinal symptoms (胃腸炎症状、81.1%) or shock or hypotension (ショックまたは血圧低下 77.4%) がみられた。

Most patients with MIS-A and COVID-19 were likely to have an underlying medical condition (持病、72% vs. 88%, respectively), but patients with MIS-A were less likely to be obese (肥満、42% vs. 60%) and significantly more likely to have received a lab-confirmed COVID-19 14 days or more before admission to the hospital (26% vs. 3%).
