欧州のデング熱 dengue は基本的に常在国からの輸入例であるが、2010年以降、国内感染及び限定的なアウトブレイクの発生の報告が増加している。2023年、autochthonous cases were reported: 66 in Italy（イタリア）, 36 in France（フランス）, and 3 in Spain (スペイン) が報告された。
We describe an autochthonous outbreak of dengue in the Paris Region（パリ地区）of France during September-October 2023.
The Study
2023年9月13日、Limeil-Brévannes, a city 15 km south of Paris 在住の36歳女性が38℃を超える発熱、倦怠感、頭痛を訴え始め、during 11-13 Sep 2023 には女性の7歳の息子も同じ症状を経験しており、発症から5日目に発疹 a maculopapular rash が現れた。さらに配偶者も9月14日から、発熱、悪寒、前頭部痛、躯幹と上肢の発疹と掻痒の症状を発症した。これら患者らは海外や国内の他の地域への渡航はなかったと報告している。
9月19日、この女性は毎日発熱が続き、嘔気と嘔吐が始まったため、パリ南部の Hôpital Henri Mondor, Créteil, south of Paris 病院救急室を受診し、血液検査で血小板数減少 thrombocytopenia (10万5 000 cells/μL; reference range 150 000-450 000 cells/μL), 白血球数減少 (700/μL; reference range 1000-4000 U/L), 肝酵素異常 alanine aminotransferase (213 U/L; reference range <35 U/L), aspartate aminotransferase (266 U/L; reference range <35 U/L), and gamma-glutamyl transferase (135 U/L; reference range <40 U/L) が認められた。 C-reactive protein was 2.4 mg/L (reference range <5 mg/L).
主なウイルス性肝炎は除外され、血清学的検査 serologic tests for hepatitis A-C, HIV, and cytomegalovirus は陰性 negative であり、Epstein-Barr virus serology については既往感染に矛盾しない結果だった。
9月22日にデングを含めアルボウイルス検査 arbovirus serologic testing, including for dengue virus (DENV) を行ったところ、IgM 抗体陽性 positive であったため、19日の採血検体をあらためて解析したところ、DENV nonstructural protein 1 (NS1) antigen in plasma were positive（陽性）, as was the real-time reverse transcription PCR (RT-PCR), which could identify DENV type 2 (cycle threshold 35), confirming the diagnosis of acute DENV infection.
発症から25日後の二次検査 a second serologic test を行い、seroconversion for DENV, IgM persistence, and apparition of IgG を確認した。
10月13日の息子と配偶者の検査 DENV serologic testing on 13 Oct 2023 でも、 IgM and IgG positive が確認された。3人はいずれも回復し、重症デング severe dengue に分類された患者はなかった...
Conclusions
... We assume that this cluster might be only the tip of the iceberg of a wider latent DENV reservoir in the Paris area due to the stable presence of the vector coexisting with viremic persons returning from endemic countries. 、以下、原文参照願います。
