2022年、the WHO から世界で新たな 10.60万例の結核 new cases of tuberculosis (TB) globally を報告されており、このうち1/3が女性の症例であるが、妊娠の有無 pregnancy status についてデータには示されていない。

だが、米国内の結核報告は少ないため、結核の妊娠中および出産後の負の影響について十分明らかにならない可能性がある。
約12万3千例の妊娠を含む複数の国からの13件の報告のメタ解析 A 2017 meta-analysis of 13 studies では、妊娠中の結核感染 TB disease during pregnancy による、with increased odds of maternal morbidity and mortality（母胎有病率および致死率）, including hospital admission（入院）, anemia of pregnancy（貧血）, cesarean birth（帝王切開）, miscarriage, preterm birth（流早産）, low birthweight（低出生時体重）, and neonatal TB（新生児結核）の増加との関連が示されている。
妊娠中は胸部レントゲン検査が避けられることから結核の診断が遅れる傾向にある。
周産期結核は、先天性および新生児結核の致死率が約50%と、致死的な疾患であり、妊娠前並びに妊娠中の結核の診断治療が重要となる。
This report describes a case of fatal neonatal TB after successful in vitro fertilization in 2021.
Investigation and outcomes
この患児の母親は、2022年の世界中の結核の27%を占めている母国のインドで体外受精を受け、出産前に米国に戻った。米国での妊婦健診で体重増加不足、悪阻、胃内容逆流によるものとされた慢性咳嗽を指摘されていた。妊娠中の一般検査は異常なかったが、結核についての検査は行われていなかった。妊娠33週に早期前期破水となり、その後合併症のない経腟分娩となり、新生児と胎盤に異常は見られなかった a healthy-appearing newborn and a normal-appearing placenta。1分後と5分後の Apgar scores of 7 of 10 and 9 of 10 であり、出生時体重は2460 g (90th percentile for gestational age であった。生後14日目に退院し自宅に連れ帰られた。
しかし、退院後まもなく努力性呼吸 labored breathing がみられ、次第に様子がおかしくなり、4日後（生後18日目）に敗血性ショックの状態でふたたび、気管内挿管の状態で集中治療室に入院した。
胸部X線像は overall ground-glass-appearing infiltrates, suggesting inflammation, and loss of lung volume であり、所見、母親に慢性咳嗽があり結核高蔓延国出身であることから、肺結核 pulmonary TB が疑われた。
患児の胃内容吸引検体 The infant's gastric aspirate samples 中に acid-fast bacilli on smear microscopy (an indicator of pulmonary TB) and grew _Mycobacterium tuberculosis_ in culture （培養で結核菌）。
生後22日目に結核治療 TB treatment** が開始され、当初改善が見られたが、結核診断から12日目に気胸 a pneumothorax was identified in the context of sudden respiratory deterioration...
The infant died on the 42nd day of life of TB-related respiratory failure.
The mother's chest radiograph demonstrated bilateral reticular nodular opacities. Acid-fast bacilli were identified on sputum smear microscopy, and a sputum sample tested positive for _M. tuberculosis_ by polymerase chain reaction; a sputum culture was also positive...
