Two HUS-related deaths recorded in Argentinian province

2024年のこれまでに，国内の小児2名 Two children have died in a province in Argentina が，溶血性尿毒症症候群 hemolytic-uremic syndrome (HUS) により死亡している。地域保健当局 The Ministry of Health of Córdoba は年初からの4週間に，7例 cases of HUS and 2 deaths が報告されており，患児らに関連性は認められていないと明らかにした。HUS は，志賀毒素産生性大腸菌感染症の重篤な合併症 a severe complication associated with [Shiga toxin producing] _E. coli_ infections で腎不全を発症する。

患児らはすべて7歳未満で，男児6名と女児1名である。4名が Córdoba の住民で, Río Cuarto and San Justo のじゅみん各1名と，1名は Buenos Aires から報告されているが Córdoba を訪れていた。いずれもそれぞれ異なる医療施設に入院した。5人は退院したが，from Río Cuarto and Buenos Aires の2名は死亡した。

From January 2023 to November 2023, 18 cases of HUS were registered in the province of Córdoba, a figure lower than the 27 cases in the previous year [2022].

_E. coli_: 2 children infected and hospitalized in the Rhône

the Rhône の18か月と7歳の女児が，大腸菌感染のため入院となっている。

18か月の乳児は2023年12月19日に the Rhône において大腸菌に感染した。the Leclerc store in Meyzieu で購入されたチーズ morbier [a type of cheese] を食べていた。その後状態が悪化し，25日間昏睡状態となり，透析が導入された。2月9日現在，意識は回復しているが，依然として深刻な状態で，食べることも歩くこともできず，左手は完全にマヒしている。

同じ時期に7歳女児も， the north of Lyon にある別のスーパーマーケットで購入した morbier を食べており，今も Lyon の病院で危険な状態であり，腎移植を受ける可能性がある。この女児も48時間の昏睡導入されていた。

Summary of the situation

2023年12月19日，

On 19 Dec 2023, the Pan American Health Organization/World Health Organization (PAHO/WHO) は，西部ウマ脳炎ウイルス感染循環による公衆衛生 human health associated with the circulation of the Western Equine Encephalitis virus (WEE) 上の注意喚起を行い，2024年1月10日に更新した。

1月9日時点で，動物の感染 1314 outbreaks(1) in animals (1258 in Argentina and 56 in Uruguay) が報告され，21例のヒトの感染例 cases of this virus in humans が確定診断され，うち1例は死亡し，いずれもアルゼンチン国内の患者であった。

2月5日時点で，2438 outbreaks in animals (1419 in Argentina, 1018 in Uruguay, and one in Brazil) and 58 confirmed cases in humans (56 in Argentina and 2 in Uruguay) となった。

Western Equine Encephalitis (WEE) cases in animals（動物）

アルゼンチンにおいて，between 25 Nov 2023 and 5 Feb 2024 に報告されたウマの感染例 the National Food Safety and Quality Service (SENASA per its acronym in Spanish) confirmed 1419 cases in equines (45 diagnosed by laboratory and 1374 by clinical and epidemiological link) which are reported in 20 provinces: Buenos Aires, Catamarca, Chaco, Córdoba, Corrientes, Entre Ríos, Formosa, Jujuy, La Pampa, La Rioja, Mendoza, Misiones, Neuquén, Río Negro, Salta, San Juan, San Luis, Santa Fe, Santiago del Estero y Tucumán. The greatest proportion was reported in the province of Buenos Aires, accounting for 29% of laboratory confirmed cases in equines (n = 13) (5).

ウルグアイでは，between 2 Dec 2023 and 6 Feb 2024, the Ministry of Livestock, Agriculture and Fisheries (MGAP per its acronym in Spanish) confirmed 1018 suspected cases in equines (77 confirmed by laboratory). The cases with positive laboratory results have been identified in 16 departments of the country: Artigas, Canelones, Cerro Largo, Durazno, Flores, Lavalleja, Montevideo, Paysandú, Rio Negro, Rivera, Rocha, Salto, San José, Soriano, Tacuarembó, Treinta y tres. The greatest proportion was reported in the department of San José, accounting for 30% of laboratory confirmed cases in equines (n = 23) (6).

ブラジルにおいて，on 26 Jan 2024, the Secretariat of Agriculture, Livestock, Sustainable Production and Irrigation (Seapi per its acronym in Portuguese) of the state of Rio Grande do Sul, reported a confirmed WEE equine case in the municipality of Barra do Quaraí, on the western border of the state of Rio Grande do Sul. The Federal Agricultural Defense Laboratory of Minas Gerais (LFDA/MG per its acronym in Portuguese) confirmed the WEE diagnosis following receipt of the sample collected on 15 Dec 2023 (7).

Human cases of Western Equine Encephalitis（ヒトの感染例）

アルゼンチン国内で初めてのヒトの西部ウマ脳炎感染例 the 1st human case of Western Equine Encephalitis (WEE) が報告された2023年12月20日以降，epidemiological week (EW) 4 of 2024 までに279例 suspected human cases among 15 provinces が報告されている。

このうち56例が検査で確定診断 laboratory confirmed され，内訳は the provinces of Buenos Aires (n = 31), Santa Fe (n = 13), Entre Ríos (n = 5), Córdoba (n = 4), the Autonomous City of Buenos Aires (n = 2), and Santiago del Estero (1) である...

確定診断された症例はすべての年齢層で確認されており，年齢の中央値は58.5歳の81歳から4か月の患者で，57%が50歳から69歳までの年齢層であり，男性が82%を占めている。
